Heat-Related Injury/Illness Survey
for UPS Teamsters Workers
There are two surveys connected to the link below. The General Information Survey is used to collect information on your related experience with Heat-Related Injury/Illness. The Heat-Related Injury/Illness Questionnaire is used to collect information about a specific heat-related injury or illness you experienced while at work. All data collected will be available to your Local Union upon request and may be used by the IBT for research and investigation into safety and health matters.
